ABSTRACT

To simultaneously determine the contents of p-coumaric acid, chlorogenic acid, 5-caffeoylquinic acid, 4-caffeoylquinic acid, caffeic acid and ferulic acid in Imperatae Rhizoma concentrated granules, an ultra-high performance liquid chromatography (UPLC) with two internal references method (TIRM) was established and validated. Chromatographic separation was achieved on a ZORBAX RRHD Eclipse Plus C18 column (2.1 mm × 100 mm, 1.8 µm) using 1.7 mmol·L-1 oxalic acid in water and methanol as mobile phase. The flow rate was 0.4 mL·min-1 and the column temperature was set as 35 ℃. The relative correction factors (RCFs) of caffeic acid and ferulic acid using p-coumaric acid as internal reference were calculated and the RCFs of 4-caffeoylquinic acid and 5-caffeoylquinic acid were calculated using chlorogenic acid as the internal reference. The TIRM was fully validated for linearity, accuracy, repeatability, stability and recovery so that it could be compared with the external standard method (ESM). The RCFs of 5-caffeoylquinic acid, 4-caffeoylquinic acid, caffeic acid, and ferulic acid were 1.069, 1.022, 1.368, and 1.493, respectively. The TIRM and ESM were used to determine the contents of six ingredients in Imperatae Rhizoma concentrated granules from different manufacturers and the variation between results was within acceptable limits. In conclusion, the newly established TIRM allowed simultaneous determination of six ingredients (p-coumaric acid, chlorogenic acid, 5-caffeoylquinic acid, 4-caffeoylquinic acid, caffeic acid, ferulic acid) in Imperatae Rhizoma concentrated granules, providing support for the quality control of this traditional Chinese medicine.

ABSTRACT

The clinical data of 2 children with refractory Mycoplasma pneumoniae pneumonia complicated with intracardiac thrombosis and pulmonary embolism, treated in Tianjin Children′s Hospital from July 2019 to January 2020, were analyzed retrospectively.One patient was an 8-year-old girl, who was admitted to the hospital mainly because of " fever for 5 days and cough for 2 days" . The other patient was a 7-year-old boy, who was admitted due to " intermittent fever with cough for 24 days and chest pain for 1 day" . Both children were complicated with elevated D-dimer levels and positive antiphospholipid antibody.During the treatment, right cardiac thrombosis and pulmonary embolism occurred in both cases, and the manifestations of thrombosis related symptoms were not typical.After the patients received anticoagulation treatment and thrombolytic therapy with urokinase, the cardiac thrombus dissipated and the pulmonary embolism improved.For refractory Mycoplasma pneumoniae pneumonia complicated with intracardiac thrombosis and pulmonary embolism, thrombolytic therapy can be applied to some children in addition to anticoagulant therapy.

At present, the situation of prevention and treatment of hepatitis B(HBV) infection in China is still serious. Factors affecting the course and outcome of HBV infection mainly include virus and hosts. While exploring the new clinical significance of HBV infection classic diagnostic indicators such as hepatitis B sunface antigen, hepatitis B core antibody and HBV DNA, more attention should be paid to the application value and prospect of new indicators such as HBV RNA and hepatitis B core-related antigen. With the proposing of the omics concept and the development of technology, people begin to focus on HBV infection with the concept of omics and integrate genomics, metabolomics, proteomics, etc., to achieve the goal of accurate diagnosis.

ABSTRACT

It is unanimously accepted that stroke is a highly heterogeneous disorder.Different subtypes of ischemic stroke may have different risk factors,clinical features,and prognoses.The aim of this study was to evaluate the risk factors,clinical characteristics,and prognoses of different subtypes of ischemic stroke defined by the Trial of ORG10172 in Acute Stroke Treatment (TOAST) criteria.We prospectively analyzed the data from 530 consecutive patients who were admitted to our hospital with acute ischemic stroke within 7 days of stroke onset during the study period.Standardized data assessment was used and the cause of ischemic stroke was classified according to the TOAST criteria.Patients were followed up till 30 and 90 days after stroke onset.It was found that large-artery atherosclerosis was the most frequent etiology of stroke (37.4%),and showed the highest male preponderance,the highest prevalence of previous transient ischemic attack,and the longest hospital stay among all subtypes.Small artery disease (36.4%) was associated with higher body mass index,higher plasma triglycerides,and lower plasma high-density lipoprotein cholesterol than cardioembolism.Cardioembolism (7.7%),which was particularly common in the elderly (i.e.,individuals aged 65 years and older),showed the highest female preponderance,the highest prevalence of atrial fibrillation,the earliest presentation to hospital after stroke onset,the most severe symptoms on admission,the maximum complications associated with an adverse outcome,and the highest rate of stroke recurrence and mortality.Our results suggest that ischemic stroke should be regarded as a highly heterogeneous disorder.Studies involving risk factors,clinical features,and prognoses of ischemic stroke should differentiate between etiologic stroke subtypes.

ABSTRACT

OBJECTIVE:To establish a method for the simultaneous determination of acetic acid and succinic acid in Banxia syrup. METHODS:RP-HPLC was performed on the column of GL InterSustain-C18 with mobile phase of 0.03 mol/L ammonium di-hydrogen phosphate buffer solution(pH2.0)-methanol(gradient elution)at a flow rate of 0.8 ml/min,the detection wavelength was 210 nm,column temperature was 30 ℃,injection volume was 10 μl. RESULTS:The linear range was 0.020 98-0.209 8 μg/ml for acetic acid(0.999 9)and 12.04-120.4 μg/ml for succinic acid(r=0.999 9);limits of quantification were 0.15,18.24 ng,limits of detection were 0.045, 5.53 ng;RSDs of precision, stability and reproducibility tests were lower than 2%;recoveries were 97.45%-101.68%(RSD=1.39%,n=9) and 98.31%-101.08%(RSD=1.01%,n=9). CONCLUSIONS:The method is accurate and rapid,and suitable for the simultaneous determination of acetic acid and succinic acid in Banxia syrup.

ABSTRACT

The violanthin, a specific component, was separated and identified from the stems of Dendrobium officinale by chromatographic technique and spectroscopic method for the first time. The microscopic characteristics of D. officinale powder were examined under a microscopy and described. Thin layer chromatography (TLC) method was used for qualitative analysis of the violanthin from D. officinale stems with a mixture of ethyl acetate, butanone, formic acid and water (4∶3∶1∶1) as the developing solvent on high performance silica gel precoated plate (SGF254) and using aluminium trichloride as a chromagenic agent. The results showed significant characteristics of violanthin from D. officinale stems on TLC, with certain specificity, and could be used to distinguish it from other easily confusing processed medicinal stems of D. devonianum, D. gratiosissimum and D. aphyllum. The content of naringenin, an active ingredient in D. officinale stems was determined by HPLC analysis on a Bischoff Chromatography HIPAK NC-04 ODS AB column (4.4 mm×250 mm, 5 mm) with acetonitrile-0.1% phosphoric acid solution as the mobile phase for gradient elution. The wavelength was set at 226 nm and column temperature was 25 ℃. The HPLC method showed good linearity within the range of 3.90-250.00 g•mL⁻¹ (r = 0.999 9) for naringenin. The average recovery of naringenin was 99.20% with 0.17% of RSD. The mass fraction of 20 batches of D. officinale stems was between 0.190 and 0.498 mg•g⁻¹. The established qualitative and quantitative method was simple and rapid with good repeatability and accuracy, providing experimental basis for improving the quality standard of D. officinale, with a very important significance to ensure its quality and clinical effect.

ABSTRACT

<p><b>BACKGROUND</b>Hypotension induced by combined spinal epidural anesthesia in parturient with hypertensive disorders of pregnancy (HDP) can easily compromise blood supply to vital organs including uteroplacental perfusion and result in fetal distress. The aim of this study was to investigate whether the goal-directed fluid therapy (GDFT) with LiDCO rapid system can improve well-being of both HDP parturient and their babies.</p><p><b>METHODS</b>Fifty-two stable HDP parturient scheduled for elective cesarean delivery were recruited. After loading with 10 ml/kg lactated Ringer's solution (LR), parturient were randomized to the GDFT and control group. In the GDFT group, individualized fluid therapy was guided by increase in stroke volume (ΔSV) provided via LiDCO rapid system. The control group received the routine fluid therapy. The primary endpoints included maternal hypotension and the doses of vasopressors administered prior to fetal delivery. The secondary endpoints included umbilical blood gas abnormalities and neonatal adverse events.</p><p><b>RESULTS</b>The severity of HDP was similar between two groups. The total LR infusion (P < 0.01) and urine output (P < 0.05) were higher in the GDFT group than in the control group. Following twice fluid challenge tests, the systolic blood pressure, mean blood pressure, cardiac output and SV in the GDFT group were significantly higher, and the heart rate was lower than in the control group. The incidence of maternal hypotension and doses of phenylephrine used prior to fetal delivery were significantly higher in the control group than in the GDFT group (P < 0.01). There were no differences in the Apgar scores between two groups. In the control group, the mean values of pH in umbilical artery/vein were remarkably decreased (P < 0.05), and the incidences of neonatal hypercapnia and hypoxemia were statistically increased (P < 0.05) than in the GDFT group.</p><p><b>CONCLUSIONS</b>Dynamic responsiveness guided fluid therapy with the LiDCO rapid system may provide potential benefits to stable HDP parturient and their babies.</p>

OBJECTIVE@#To evaluate the correlation of chronic periodontitis in tropical area and IFN-γ, IL-10 and IL-17 levels.@*METHODS@#One hundred and forty-eight patients and one hundred and thirty-two healthy control subjects were included in the study. Clinical parameters (PI, GI and PD) and GCF levels of IFN-γ, IL-10 and IL-17 were measured at baseline, week 8, week 16 and week 24 after mechanical removal of dental plaque. IFN-γ and IL-10 were determined with ELISA methods and IL-17 was determined with the cytometric bead array.@*RESULTS@#Removal of dental plaque resulted in improvement in all clinical parameters. Meanwhile, GCF IL-17 declined to control levels, while GCF IFN-γ and IL-10 levels remained unchanged.@*CONCLUSIONS@#The decline of GCF IL-17 levels in patients with resolution of periodontitis suggests that IL-17 is involved in the periodontal inflammatory process.

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the therapeutic efficacy and safety of liver transplantation for patients with cholangiocarcinoma.</p><p><b>METHODS</b>According to the requirements of Cochrane systematic review, a thorough literature search was performed in Pubmed/Medline, Embase and Cochrane Central Register electronic databases ranged between 1995 and 2009 in terms of the key words "liver transplantation", and "cholangiocarcinoma" or "cholangiocellular carcinoma" or "bile duct cancer". And restricted the articles published in the English language. Two reviewers independently screened the studies for eligibility, evaluated the quality and extracted the data from the eligible studies with confirmation by cross-checking. Data were processed for a meta-analysis by Stata 10 software with 1-, 3-, 5-year survival rates and incidence of complications.</p><p><b>RESULTS</b>A total of 14 clinical trials containing 605 patients were finally enrolled in this study. The overall 1-, 3-, 5-year pooled survival rates were 73% (95%CI: 0.65 - 0.80), 42% (95%CI: 0.33 - 0.51) and 39% (95%CI: 0.28 - 0.51), respectively. Of note, preoperative adjuvant therapies (OLT-PAT group) rendered the transplanted individuals comparably favorable outcomes with 1-, 3-, 5-year pooled survival rates of 83% (95%CI: 0.57 - 0.98), 57% (95%CI: 0.18 - 0.92) and 65% (95%CI: 0.40 - 0.87), respectively. In addition, the overall pooled incidence of complications was 62% (95%CI: 0.44 - 0.78), among which that of OLT-PAT group (58%, 95%CI: 0.20 - 0.92) was relatively acceptable compared to those of liver transplantation alone (61%, 95%CI: 0.33 - 0.85) and liver transplantation with extended bile duct resection (78%, 95%CI: 0.55 - 0.94).</p><p><b>CONCLUSIONS</b>In comparison to curative resection of cholangiocarcinoma with the 5-year survival rate reported from 20% to 40%, the role of liver transplantation alone is so limited, but neoadjuvant radiochemotherapy combined with liver transplantation can bring better short- and long-term prognosis.</p>

BACKGROUND: Current research of mechano growth factor (MGF) mainly focuses on the muscles and nerve damage and repair, and it has bean confirmed that MGF can promote muscle cell hypertrophy and nerve repair significantly. Regarding its role in fracture healing is unclear. OBJECTIVE: To investigate the effect of MGF on radial fracture healing in rabbits. METHODS: By using random digital table method, 12 New Zealand rabbits were divided into 3 groups: blank control group, low-dose MGF group and high-dose MGF group. The models with 5 mm bone defect were produced in the middle of the left radius in rabbits. At 3 days after the surgical operation, the defective areas were given 0.2 mL PBS or 0.2 mL MGF (0.36 and 0.72 g/L) injected into the ends of fracture areas, respectively, once per day for continuous 5 days. At 4, 6, 8 weeks after operation, X-ray photography was used to evaluate the healing of fracture, and the histological examinations were performed at the 8~(th) weak to observe the call morphology at the fracture lesion. RESULTS AND CONCLUSION: At 1 day after operation, the activities of rabbits were reduced, with slightly reduced food intake, at 2 days they almost recovered to normal activities and diet. At 3 days, the surgical incision slightly swelled with a small amount of bleeding and without obvious signs of infection. All 12 rabbits entered the final analysis. X-rays showed that two fracture ends have basically combined in the high-dose MGF group at 4 weeks post-surgery, cortical bone was continuous and fracture lines were unclear. At 6 weaks the bone medullary cavity almost run through and fully run through at 8 weeks. The healing time in the high-dose MGF group was remarkably shorter than that in blank control group and low-dose MGF group, the healing was in high quality. At 8 weeks after operation, a large number of osteoid tissues were observed in the blank control group, a small amount of woven bone formed, at a transition period from the fibrous bone callus to the bony bone callus; a large number of woven bone formed inthe low-dose MGF group, at bony bone callus period; in the high-dose MGF group, a large number of woven bones converted into mature lamellar bone, at the callus rebuilding phase, which was consistent with imaging results. It is indicated that MGF can accelerate fracture healing significantly in a rabbit model and shows a dose-dependent manner in a certain range.

ABSTRACT

Objective To investigate the changes of early and delayed washout rates of 99Tcm-methoxyisobutylisonitrile (MIBI) in ischemic heart disease (IHD), and to explore the value of 99Tcm-MIBI SPECT in evaluating impairment of ischemic myocardial cells. Methods Patients diagnosed of IHD with three-vessel stenosis ( ≥50% ) without myocardial infarction based on angiography (CAG) underwent 99Tcm-MIBI static planar and gated SPECT imaging. The early (90 min after the intravenous injection) and delayed (4 h after the intravenous injection) washout rates of 99Tcm-MIBI and left ventricular ejection fraction (LVEF) of IHD patients and normal subjects were compared using t-test. Linear correlation analysis was performed between the early, delayed washout rates and LVEF measured by gated SPECT. Results Statistically significant lower early washout rate of 99Tcm-MIBI was observed in IHD group than control group: (13.44 ± 2.87 )%vs ( 17.32 ± 4.92) %, t = 2.384, P < 0.05, but higher delayed washout rate of 99Tcm-MIBI was observed in IHD group than control group: (19.24 ±4.71)% vs (15.23 ±3.81)%, t= -2.246, P<0.05. LVEF in IHD group was significantly lower than that in control group: (55.71 ±7.97)% vs (67.75 ±5.43)%, t =-4.418, P <0.01. There were no correlations between the early/delayed washout rates and LVEF, respectively in IHD patients (r = -0.212, P > 0.05; r =0.352, P > 0.05, respectively). Conclusion 99Tcm-MIBI washout rate may reflect myocardial cell impairment due to IHD.

ABSTRACT

Objective To explore an algorithm to define the threshold value for tumor contouring on 18F-fluorodexyglucose (FDG) PET imaging. Methods A National Electrical Manufacturing Association (NEMA)NU 2 1994 PET phantom with 5 spheres of different diameters were filled with 18F-FDG. Seven different sphere-to-background ratios were obtained and the phantom was scanned by Discovery LS 4. For each sphere-to-background ratio, the maximum standardized uptake value ( SUVmax ) of each sphere, the SUV of the border of each sphere ( SUVborder ), the mean SUV of a 1 cm region of background (SUVbg) and the diameter (D) of each sphere were measured. SPSS 13.0 software was used for curve fitting and regression analysis to obtain the threshold algorithm. The calculated thresholds were applied to delineate 29 pathologically confirmed lung cancer lesions on PET images and the obtained volumes were compared with the volumes contoured on CT images in lung window. Results The algorithm for defining contour threshold is TH% = 33.1% + 46.8% SUVbg/SUVmax + 13.9%/D ( r = 0.994) by phantom studies. For 29 lung cancer lesions, the average gross tumor volumes ( GTV ) delineated on PET and CT are ( 7.36 ± 1.62 ) ml and (8.31 ±2.05) ml, respectively (t = -1.26, P>0.05). Conclusion The proposed threshold algorithm for tumor contouring on PET image could provide comparable GTV with CT.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the cause of liver failure after hepatectomy for hepatocellular carcinoma and explore its prevention and treatment.</p><p><b>METHODS</b>The clinical data of 1000 patients with hepatocellular carcinoma undergone hepatectomy from July 2000 to June 2008 were retrospectively analyzed. There were 922 male and 78 female, aging from 21 to 89 years old.</p><p><b>RESULTS</b>Among the 1000 patients, there were 18 patients with liver failure after hepatectomy (1.8%). Among the 18 patients with liver failure, 13 patients were more than 65 years old, 14 patients were with more than 20% of indocyanine green retention rate at 15 minutes, 14 patients were with more than 1000 ml blood loss during operation, 6 patients were with F4/F3 liver fibrosis (Metavir Scores), and 9 patients were with less than 40.0% liver volume of residue liver.</p><p><b>CONCLUSIONS</b>Patients with hepatocellular carcinoma with less than volume of residue liver, much more blood loss or transfusion, more than 20% of ICGR15, F4/F3 liver cirrhosis are prone to be with liver failure after hepatectomy. Artificial liver or liver transplantation may be the important alternative for liver failure after hepatectomy.</p>

ABSTRACT

Objective To study the prevalence of human immunodeficiency virus(Hiv),sexually transmitted infection(Sti),risk behavior and the sexual neworks among men who have sex with men (MSM)in Taizhou city,Zhejiang province.Methods cross-sectional Astudy was applied with venue-based sampling in 2 MSM gathering sites in Taizhou.'Informed Consent'principle was applied and MSM were studied through a structured questionnaire.Blood samples were collected from thee who accepted free and confidential HIV/STI counseling and then tested for HIV,syphilis,HCV and HsV-2 antibodies with ELISA.HIV positive sera were certified with western blot.Results 106 MSM were investigated and 97 qualified questionnaires were collected.25.0%(23/92)of these MSM have ever had l female sex partner while 47.8%(44/92)had 2 or more.14.3%(13/91)of them reported having had 1 male partner who had engaged in anal sex and 80.2%(73/91)had 2 or more.22.1%(19/86)of therh had partieipated in group sex but 62.5%(55/88)of them did not always use condom when having analintercourse.15.1%(14/93) of them had l orfll sex partner while 75.3%(70/93)having 2 or more.38.9%(37/95)of them had sex with female sex worker,and 35.5%(33/93)had sex with male-to-male sex worker.15.3%(13/85)0f them had once been male-to-male sex worker themselves.3.9%(3/77)of them were found HIV positive in blood tests,with 24.7%(18/73)positive of syphilis,15.1%(11/73)positive ofHSV-2 but HCV appeared to be negative.46 csses reported their egocentric recognition networks,with mean degree of 5.91(ranging0-10),and mesa density of 0.548(ranging 0.000-1.000).43 sexual networks were identified,with mean degree of 2.70(ranging 0-10),and mean density of 0.246(ranging 0.000-1.000).Conclusion Risk behaviors,such as multiple sex partners,IOW proportion of condom use and commercial sex engagement both with heterosexuals and homosexuals,were extensively existed among MSM in Taizhou,and the prevalence of HIV/STI was relatively high.Their sexual networks seemed complicated but there might be in place of some sukstantiaUy isolated MSM groups with high risk of Hlv/sTl infection.More study should be applied to identify the relationship between scxual networks and HIV/STI transmigsion.

ABSTRACT

OBJECTIVE:To prepare multivesicular liposomes-sodium polymannuronate microspheres(MVLs-Alg)for protein drugs and to study the physico-chemical property and elementary stability of MVLs-Alg.METHODS:MVLs and Alg were used as matrix and BSA as a model protein drug,MVLs-Alg microspheres were prepared by interior/gelatin method.The relationship between MVLs and Alg was analyzed by differential thermal analysis and the effect of temperature(4,25,40 ℃)on entrapment efficiency and peroxide value of phospholipid in base material was investigated.RESULTS:The MVLs-Alg microspheres were spherical.Analyzed by DTA,it was considered that physical adsorption between MVLs and Alg was potentially existed.Storing for 90 days at 4,25,and 40 ℃ respectively,the encapsulation efficiency of the MVLs-Alg microsphereswere were 85.36%,63.75%,and 50.49%,respectively.The peroxide value of phospholipid increased increasingly,but not significant as compared with MVLs.CONCLUSIONS:The stability of MVLs-Alg microspheres was heightened compared with that of MVLs and which can be used for the delivery of protein drugs.

ABSTRACT

<p><b>OBJECTIVE</b>To study the chemical constituents of Arenaria kansuensis.</p><p><b>METHOD</b>Column chromatographic techniques were applied to isolated constituents. A combination of physico-chemical properties and spectroscopic analysis were used to identify structures of constituents.</p><p><b>RESULT</b>Five compounds were obtained and elucidated as tricin (1), tricin-7-O-beta-D-glucopyranoside (2), tricin 4'-O-beta-guaiacylglyceryl ether (3), isoscoparin (4) and isovitexin (5).</p><p><b>CONCLUSION</b>Compound 2, 3 and 5 were isolated from A. kansuensis for the first time, compound 3 was obtained from the family Caryophyllaceae for the first time.</p>
